  1. OMG!  What did you do?

It could mean that it broke off - which happen to my uncle then calcified and was causing considerable pain.

Or, it means it ripped open and is leaking the jelly like substance.  It is usally referred to as a ruptured disk.

  2. This is usually referred to as a herniated or ruptured disc. The discs themselves are made of a gel-like material surrounded by a hard shell. They serve the purpose of cushioning the vertebrae, and sometimes due to injury to excessive wear, the outer shell bulges or ruptures (tears), allowing the gel to enter the spinal cord and press against the nerves.
