Question:

I have eating disorder, am 101lbs, about to get deployed, what form do I need to bring to my doctor? 514? army

I need to get treatment and I need to obtain a form to become a 'no go' so I can get that treatment. I am 101 lbs and going to lose more. I need help. What form is this that I need to give to my doctor. Is it called a 314 or 514? where would I be able to get it?

  1. If you had this eating disorder when you enlisted and did not include it in your medical history, you won't be going anywhere except out the door with a OTH or Dishonorable discharge for lack of full disclosure or omission of a mental disorder.

    Court martial and jail time are also a possibility.  


  2. ok so you have this eating disorder, and all the sudden when your fixing to get deployed you need to get a paper so you wont have to go. honestly if you have such a problem you should have gotten treatment along time ago. more than likely you wont be in the army anymore....  

  3. Eat some MREs, get on the rotator, do your time downrange and start pulling your own weight (no pun intended...okay..kind of intended) or just get out of the military. If you have gone through all of your military training and still don't have the personal discipline to control yourself then perhaps its time to become a civilian again.
