  1. Sounds like your plant is suffering from Cercospora leaf spot (see link), which stunts bud growth  This is a common disease for the plant.  Apply nitrogen, remove dead leaves.

  2. what kind of hydrangea?... the white kind prob isn't ready to bloom yet.... the mopheads, the big snowball in blue or pink should have bloomed, but a lot of folks suffered a frost early this spring that hurt the buds of their hydras.... go here, find out what kind you have..

  3. how old is the plant?  the brown spots either mean to much watering, or mold.  1 part dish soap to 2 parts water will fix that.

  4. Depending what zone you live in. Hydrangea grow best in zones 8-9. They are temperamental especially when it comes to night time temperatures. They don't like it to go below 60 degrees Fahrenheit and will not perform well if temperatures go below 60 too much at night during blooming season.  One of the first things they will do when not performing is what you described. The second is sterile plants with no flowers and 3rd is dying off. I have repeatedly tried to grow hydrangea in zone 7 and have found them to be difficult.
