I have lost ma pancard and had given the details of the last residence and office now i dont have both wht do?

i lost ma pancard i do know the pan card no. but the problem is that the address proof was of ma last residence and office where i am not now living and working so how to get the new one of the same no. and the same pan card ma no. was botps6124m.....plz do reply.....

go on the official site for the IT department which is http://www.incometaxindia.gov.in/

go on the link PAN on the leftside of the web page click on CHANGE PAN n fill in the form with the new details mark the fields that uve changed submit the form with the required money at one of the centres close to ur place n a new card with the changed details will be issued to u...dont forget to attach the residence proof of the new address.

in case u want a print click on the link KNOW UR PAN link..key in ur details in ull get ur pan no.
