I have lost my ecomomic stimulus payment check. What can I do next ?

download Form 3911 from the IRS website www.irs.gov and fill out the form and mail to appropriate IRS address.

Call the IRS and report it lost.  They'll send you an affidavit for your signature.  Sign it and return it quickly.

Plan on 8 to 12 weeks total for a replacement check.

You can complete Form 3911 and write Stimulus across the top.  You can find the form at the IRS web site, www.irs.gov in the forms section, near the top on the left side.  Also, you should send the form to the IRS office where you file your return.  Doing this cuts out calling the IRS and waiting for them to mail the form to you.

Call the IRS at 1-800-829-0582, extension 726, and they will initiate a trace. If it turns out the check was cashed, they will send you a photocopy and a form to fill out stating it wasn't you. If they agree, a check will be reissued. If no one cashed the check a replacement will be sent.
