I have lost my policy, is there a way that I can get a copy?

I also would like to know if my wife and myself can be put on the same invoice of bill? If so please do so. I also would like to know why do I only have a $5000.00 policy and not the $10.000 life insurance like I ask for. Could it be because of my health problems?

Call your insurance carrier, they will be able to supply a copy to you. And for the other questions you still need to contact your insurance carrier since al carriers do not have the same guidelines.
