  1. If I'm not wrong, you just need a passport and at your arrival here in Dominican Republic you have to pay 10 us/p.p. and at departure 20 us/p.p. Have fun !, and happy to help you!

  2. Where you live and where you are a permanent resident does not matter - the important thing is what country you are a citizen of.  You need to find out (check DR consular web sites) to see what the requirements are for your country of origin/citizenship.

If you are a permanent resident in Canada, you need a passport  from your HOME country to travel internationally.  If you want to travel to the Dominican Republic and are asking if you need a visa to travel there. you can see by using the site below. Just enter your nationality, enter that you are an alien resident of Canada, enter your destination and click for the info.  Your student visa in the U.S. has nothing to do with  travelling to other places.
