I haven't played trombone in years, what's a good method to relearn how to play?

I'm looking at playing trombone in a local jazz band. I used to be pretty good 4 or 5 years ago, but haven't really played since. I picked it up today and was just horrendous. I remember slide positions (for the most part), I just couldn't get my mouth to do what I wanted it to. My "chops" are seriously out of shape. Sexual innuendos aside, does anyone know a good method to get me blowing again?
