It's a reference to the size of the genitals.  It is usually referred by someone comparing their genitals or the genitals of someone they know or have seen to those of a horse.  It means that they are very large.  i.e. "Hung like a horse".

They have a large p***s, the stallion or gelding that is. So not all horses hang well, mares for example. They are also herd animals and are happiest in a group. Perhaps overall horses hang, as in hang out with others, well. But only stallions or geldings may be well hung. Unlike with men, horses size does vary that much, only when referenced to the actual size of the horse. Example, a 17.2hh, 1300 lbs gelding will have a larger p***s than a 13hh, 750 lbs pony gelding, though when compared to their body size the p***s will be the same size.
