  1. Surname? pfffffttt. He was "just Lurch". Surnames weren't abundant on the show. Addams was the only name they worried about.

  2. He had no first or last name.  You see he was left in the Lurch.

  3. Short Answer: Lurch

Explanation:  When cartoonist Charles Addams originally created the character of the manservant for the Addams family cartoons that appeared in "The New Yorker" beginning in the 1930s, he followed the well-established tradition of calling the valet or butler by his surname.  For example, 1920s and 1930s English authors P. G. Wodehouse and Dorothy Sawyers respectively called their valets "Jeeves" and "Bunter", even though their full names were Reginald Jeeves and Mervyn Bunter.

In episode #17, "Mother Lurch Visits the Family",  of the 1966 to 1971 television series, Mom calls Lurch "Sonny", but she doesn't let on  whether this is his first name or his nickname.  Ted Cassidy (1932-1979) played the manservant in the series.
