  1. If she's still in Kuwait, tell her to visit the SSS office there. It is easier than visiting the SSS offices in the Philippines.

SSS Representative: Mr. Francisco U. Uy

Address: Embassy of the Philippines, Area 6, St. No. 103, Villa No. 503, Jabriya, P.O. Box 26288 Safat 13123 State of Kuwait

  2. just go to any SSS branch, give her name and birthdate, they can give you her SSS number on record.

  3. just go in any SSS offices and request for the new idea, they have a form to fill up.

  4.    First, ask your mother - what is the name she used to register in the SSS - is it her maiden name or her married name.

Go to any SSS on-line branch, bring your mother's birth certificate issued by the NSO or the civil registrar of the city or municipality where was born.  If she used her married name, bring her marriage contract.

SSS will search their database based on your mother's date of birth and the name she used in her REGISTRATION ( maiden or married name ).

Reminder :  Do NOT ask for a new SSS number - past contributions your mother made will not be credited to a new SSS number.

       Have a nice day !
