I lost my prescription slip from hospital, now cannot get my medicine?

I was given a tiny slip of paper with which to get my repeat prescription medicine. This was given to me by the hospital consultant, not my own doctor. I have been a bonehead and lost the slip of paper. Is there any way the pharmacy at the hospital would give me my medicines without the skip? I ask because I deal with the NHS in the UK, and if I have to get a new hospital appointment with the consultant this could take months, which I cannot wait that long. Any advice from anyone, whether working in NHS or not, would be appreciated. Thanks.

  1. Simply ring the hospital, ask for the secretary of the Consultant that you saw and explain what has happened. She will arrange for either the consultant to issue a new prescription or she may organise it to come from your GP.


  2. The hospital pharmacy in the UK will always need a  note from your physician before issuing any medication there is no option to turn up looking for a medication without it.

    In the community pharmacies it depends on the type of medication.. If it is one you have had before, same does same name  and not open to abuse then the pharmacist  has the discretion to issue an emergency supply.

    Alternatively you need to contact the hospital secretary and ask for confirmation to be faxed or discussed with your GP practise and they can issue another prescription if appropriate..  

  3. Call the consultants office and arrange for either a new perscription or have them call the pharnacy with the details. You will need to take proof of ID.

  4. Hospital usually advise your doctor of results of tests.  diagnosis  and medication prescribed.    So he should be able to    give you  a new  one  

  5. if you ring your consultants secretary she will be able to organise a repeat for you or give you the appropriate advise when i did the same my consultant got my gp to issue a new prescription as he had been advised of the drug change

  6. hi the only thing i can suggest is to call the consultants receptionist to c if they will give you a copy of it  

  7. Phone the hospital and ask to speak to the Consultants secretary. She will have access to your file and can take it from there. Or your GP  by now might have received a letter from your consultant detailing what he has prescribed.

  8. As long as you know what the medication was, it shouldn't be a problem. You fill out a repeat prescription slip request and give it to the pharmacy. They should have the slips available.

  9. You may ask them to give you the copy rite?

    As i know when they write a bill there will be one more page they keep for themselves.
