I lost my social security <span title="card/citzenship/naturlization">card/citzenship/naturliza...</span> forms-what do I do now?

I lost my social security card and I also lost my proof of citizenship as I was not born here in the US but was naturalized in 1996.

What agencies/offices do I go to obtain a new social security card and get a new naturalization/citizenship form?

  1. You need to use form SS-5 and go to a local Social Security Office to obtain a replacement Social Security Card.

    You need to use form N-656 to replace a Naturalization or Citizenship document. This form is mailed into the USCIS Service Center with jurisdiction over your area of residence. More information can be found in the link below for N-656.
