We made small talk and I knew without a doubt that we have that rare and very special chemistry but I had been sick all week and that was my first time out after being sick and I just didn't have myself together. What should I do?

when you feel better, go back in the restaurant and sit with her in a table, tell her how you feel. if you cant do that, try getting her number and talk to her on the phone.

Part of waiting tables is playing up to customers, and this quite often involves innocent flirtation, especially if the person is there alone, or an obvious "extra" at a table of friends.  

This isn't to say that she isn't interested in you, but to caution you against thinking some small amount of flirtation is a bona fide interest when it could just be her schmoozing for tips.

If you think there's a real interest there, ask her if she'd like to meet you after her shift at a nearby coffee shop or something.  If you're not sure if it's a genuine interest, you can always try stepping up the game a bit by giving her direct compliments, like telling her that she's interesting.
