  1. where you born in ohio...if  so go to the court house and you can get one on the spot

  2. you can get one through the mail in about 4-6 weeks or go to wherever your state holds the records and get one that day.   i'm in virginia, and i had to drive to richmond and got one that day for about $12

  3. You have to call the county where you were born and answer some questions about who your parents are and some other info, then they can mail you one. It cost me about $20 total with shipping (I had it expressed mailed because I needed it quick).

  4. Roughly $20 in administrative fees.  Roughly 4 to 6 weeks.

  5. once i lost mine and my mom got it for me right on the spot.

  6. It is super easy if you were also born in Ohio.  Just go to your local health department, and for $15.00 they can print a new one for you right on-the-spot...

At least that's what they do in Missouri...

call ahead and ask!   Good Luck!
