I need a chart?? How & where??

Question

I need a chart ASAP that looks like this one.

It just needs to be bigger. I don't have wordpad or anything. But I need this so if you know where I can get one that would be AMAZING !.

Do you want the chart just as a blank picture like in the link, or do you want it so that you can fill in info inside it?

EDIT - OK. So you want such a chart which will enable you to fill data in the chart. To do this simply open Microsoft Word.

First of all do you have Word? If no, then you should download a program called Open Office and try out what I'll tell to you in it.  http://www.openoffice.org/

I have Word 2007 and so I'll tell from it's viewpoint. After you open it, go to the Insert tab and click on Table. You will be able to make your form you mentioned with its help. Then just select Draw Table manually and you'll be able to draw a table of your choice. This is very handy. Or you could even just click on Make a Table and select the number of columns to be two and rows to be as many as you want. Then, just drag the line dividing the two columns to the left as per your wish.
