I need a copy of my s.s card and birth certificate, what do i do to get one?

i'm 16 and i need a job, i can't get one unless i have proof that i am legally elligable to work in the u.s. when we moved my momma lost my birth certificate and social security card. I live in KY and i need to find a website where i can print off the forms to get them both so i can fill them out and mail em' to frankfort. Does anybody have and website suggestions?

  1. Get a copy of your Birth Certificate  direct from state and county .  Vital Records

  2. Usually you go to the county records office for a birth certificate, county in which you were born, they will tell you what kind of information and documentation you will need to send to them, to get a new one, probably a small processing fee too.

    You can get the forms at the offices...you might as well get them there...b/c you will have to wait in line anyway.

  4. You get the birth certificate from the records office in the city you were born in, and that office is called different things in different states.  In Saint Louis it is handled by the County of Saint Louis City (yes, Saint Louis City is a county!!!!).

    Social Security card you need to go to the Social Security Administration office nearest you.  If you do not now have a Social Security Card take your birth certificate when you get one to the Social Security office and file for a card.  You have to have the birth certificate first though.
