I need a get a copy of my shots record?

I'm 19 and I need my shot records to get admitted to U of M. I can't find them anywhere. Please Help!!!

ask your mom where you got vaccinated at, the Dr office will have records, there should also be copies of those where you attended grade school.

if SC (i live in south carolina) you have to go to the health department in the county that you were born in. so if you were born say in orlando fl, you'd have to find the number and call their health department and they can most likely mail them to you. or you can just call your health department in the state you're living in now and see if they can help you.
