  1. The only fee you'll see is the cost of the card.  A $50 card might cost you $5, give or take a buck.  Though that money doesn't go to the casino.  Also, make sure you keep the card, as some sportsbooks require a copy of the card when you pay out.  If you mention it's a gift card, you shouldn't have a problem, but it's better to be safe than sorry.  As much of a pain it may seem, the casinos want to make sure it's YOU that gets the money and not an identity thief.

I think All Access is coming out with a reloadable card, but I think you're just better off buying a new card if need be.  There should NOT be any hidden fees on the sportsbook's behalf if you're using a prepaid card.  I would recommend a site like Intertops.  Check http://dagus-statistics.com/reviews.htm for a list of sites that accept prepaid cards (if they accept Visa, then a prepaid Visa will work, ditto for Mastercard).  You'll also find great bonus codes, as well.

Although, be aware that "Vanilla" and "Green Dot" prepaid cards do not work, their company filters out online gambling transactions.  I would fully recommend Visa All Access Gift Cards.  I've used them on numerous sites.  Just make sure you wait at least a half hour after you buy one to deposit, they have a company waiting limit for some reason post activation.

Anyway, navigate around the site listed above, it should answer all your questions about online gambling.  If not, feel free to e-mail the webmaster.  Hope this helps.

  2. There should definitely be no fees other than maybe something for the credit card. At many sites they will actually add a bonus to any first deposit, meaning you will get a more than $50. Checkout http://www.fullofpoker.com for a sportsbook bonus offers
