Do you not have the back on your form? Because on the back (or page 2)  of Form 4506-T Request for Transcript of Tax Return; it clearly states if you live in Arizona you fax the Form 4506-T to 559-456-5876. Or mail to:  Internal Revenue Service RAIVS Team Stop 37106 Fresno, CA 93888.
