  1. you can just do it online ..  here hope this helps

  2. go to the stadium

    take a left on 3rd

right on to Martin luther king dr.

then go straight untill u are behind the chicken &waffle place

talk to my man pedro and he will hook u up with all u need

  3. Try the vital statisics office in Berwyn.

  4. You need to call the City Hall of the city you were born in. They are the ones that hold your original birth records. I know you can get a certified Birth Certificate on-line from AL - not sure about IL though. Start with the clerk's office at City Hall - if they can't help you they can give you info about where you need to go,

  5. For birth records of newborns (6 months old or younger) born Chicago, call the City of Chicago's Health Department at (312) 744-3800. For birth records of newborns (6 months old or younger) born in suburban Cook County, call the Cook County Clerk's Current Records Unit at (312) 603-6623.

You can even order a birth certificate from the Internet.  How convenient is that?

    Check out the website below.

  6. Go to the local courthouse they will have a room where you can request a new one. I would bring other proof that you are you with, SS card. They can give it to you or send it over to the county you live in. There might be a small fee.
