I put a tampon on and i realized i was leaking and i want to know what could it be.Did i put it on wrong ?

It could be one of two reasons, 1-you inserted it wrong or 2-wrong absorbency. You didn't say if it was full or not.

You may not have inserted it properly, try again and if it happens again change brands!

if the tampon was comfortable, then u put it in right.  if it leacked through, then the size was wrong or u just have an extra hevy flow right now.  always put on a thin pantyliner for a back up and check on it often when ur flow is heavy.  change ur tampon at least every 4 hours when it's heavy.

you probably just got the wrong size tampon.

when you pulled the tampon out was it full? if so then it needed to be change because it was to full and that's why it happened, if that's the case then try a higher absorbency,

if not then most likely you put it in wrong, in the box of tampons there should be directions look at it because usually theres a picture
