2 things:

If it's a directv box, there will be a little red button in the same place that your access card is pushed into the slot.  Make sure your card is in there good.  With the unit connected to power, press the red button and reboot the system.  this usually fixes these types of problems.

And don't go buy an upgrade for a directv box.  They are all considered leased and if there's a problem they will most likely send you out a refurbished unit (i haven't had a problem with my refurbished unit) to replace the faulty machine.  It's a hassle, but it'll cost a lot less than buying a new machine.

Your hard drive is most likely dead on your DirecTivo.  Go to dvrupgrade.com and either buy a replacement hard drive or buy an instantCake CD for $19.99 and fix it your self.
