Okay so I want to add a middle name, as I don't have one. I'm renewing my passport right now and I'm wondering if I should just add the name on the form or use a deed poll?

  1. you can't just add a name to a passport you will need to change your name officially but in saying that call your local registry office, they will keep you right.

  2. You don't say which country your from, but the term deed poll makes me think England.

If you want to renew your passport, and have a name on it that wasn't in the previous passport, then you will need to supply a deed poll document with the application, along with your birth ceriticate.

Basically they need to see a paper trail from your birth name to the name you are now using.

More information can be found in the link below (applies to UK passports - may not be applicable, but you've not said where you're from).

  3. You definitely need some written evidence of the legal addition of a middle name attached, which means you need to go through an official name change procedure before submitting any paperwork for a new passport.

  4. do not add anything to your passport yourself

only the guys at the passport office can do that

and you will need to change your name by deed poll

    otherwise its not legal

and the passport will be invalid
