First choose name of your choice.

Now go to local court (Collectorate) of your District.

Contact a Notary.

Prepare a ddeclartaion (affidavit) on Non-Judicial Stamp in consulatation with Noatry of the effect that you have changed your name from A to B.

Go to office of some newspapers and make a same declaration in those fro public information.

Always keep a copy of that affidavit and produce it whenever you are giving your name in any form, document, etc.
