Print this form and fax or mail it. These requests must be made to the local (Toronto) branch of the Ontario Office of the Attorney General. This is the office which handles Birth, Death etc Certificates and records. They do not allow access to records over the Internet. You have to request these by mail or fax. Do the same for Quebec.
