  1. There is only one way in the US and Canada and it requires you to go to court, pay to publish the name change and get the other parent to consent in court to the change of name.

On the other hand, schools will let you enroll your child using the name on their birth certificate, but get called by whatever last name the guardian or parent uses. So if you're Jane Logan and your daughter is Marielle Quiggley, she can use the name Marielle Logan in school and to all of her friends. I'm amazed at how many kids show up on graduation programs with a name no one ever knew. "It's my dad's name, but I don't use it." is such a common answer. That way is free and your child can make it permanent later when there's more money.

  2. This is a legal question, not a genealogy question.  The laws of this vary by state, and the fees vary by county.  In all cases, the only legal way is through the court.  Contact your county courthouse for the laws and fees in your area.

  3. You should contact the legal aid people in the nearest big city to you. If you already live in such a place, just telephone or go downtown and get that information. Sometimes, it is totally free to those who can't afford to make that change. There will be some documentation required, which of course, is all legal.

  4. UK answer.

Here in the UK, you can now change a name by deed poll at the register office where the birth was registered, it cost's around £40:00 and there is no need for a solicitor.
