  1. Well, I would start with your local State Board of Nursing since they are the ones who issue licenses.

    I think having your license reissued will all depend on the reason why you lost it in the first place.

    Many states only judge the issuing of licenses on 10 years of history, so perhaps the time has passed for any denial of a new license.

    I am a nurse in Arizona, and have attended meetings of the State Board. They are pretty tough on CNA licensure here, and again, I would start with the agency who issues the licenses, or just go ahead and re-apply.

    They must have advised you at the time your license was revoked, what the 'punishment' was for your violation and when you would be eligible to have one re-issued..?
