  1. Contact the consulate in Winnipeg.

    Winnipeg office

    Phone:

    (204) 940-1800

    Address:

    201 Portage Avenue, Suite 860

    Winnipeg, Manitoba

    R3B 3K6

  2. Are you a US citizen?  What are you doing in Canada without your passport?  If you need a replacement passport for one that you lost, contact the nearest US consulate.  I believe there is one in Winnipeg.

If you do not have a prior US passport, you probably have to get back to the US to get an original passport.  Ask at the US consulate if they can issue original passports.  You'll need your birth certificate and additional US-government issued photo identification, and perhaps proof of US citizenship or residence in the US from the time of your birth if your parents were not US citizens at the time of your birth.  If you were born outside the US, you need proof that your parents were US citizens or proof of naturalization.

  3. Any US embassy.

  4. http://travel.state.gov/ . . . third column is information for foreign applications . . .

  5. There should be an U.S. embassy in MB or around there that you can apply to get your passport.  Otherwise you may just have to mail it to a U.S. embassy elsewhere in Canada.  Just look up U.S. embassy office's in Canada.  that should be a start
