  1. It all depends on your Nationality.


  2. You will need a passport for sure, I don't think you need a visa if you are a permanent resident.  You might be able to find some info on the government of Canada website.  

  3. for UK you'll need a passport, but depending on your home country if you need a visa, so check their govt website: http://www.ukvisas.gov.uk/en/

    for USA you'll need to apply for a visa 3 days before entering as the govt recently made these changes to help protect their borders.  check this site: http://www.unitedstatesvisas.gov/
