Yes - mostly.  Once you get into upper management levels, you'll probably start getting salary.

Hourly pay is usually customer support/service type work.

Depends on your position. Clerical positions are typically hourly. Claims personnel, actuaries, underwriters and management are typically salaried.

Depends on the position.  Support and clerical are generally non-exempt, thus hourly.  

Higher up the chain and you get salary.
