you are right to be confuse, but remember all the other places next to los angeles and you will get a very very large metropolitan area,  belive me if not try google earth and you will see how small is PVR compare to LA and LA metro area.

the City of Los Angeles is closer to 470 sq mi, Puerto vallarta is almost 503 according to Wikipedia.

Don't confuse the City of Los Angeles with the County of Los Angeles; many people think they're the same thing.

That said, Puerto Vallarta is a VERY large metropolitan area...It's much more than just the tourist areas on the coast.
