  1. It is just counted as an out, because errors are not something the batter can influence, and therefore, they do not get credit for them.

  3. Utter chaos is correct. Reaching base on an error does not count toward a batters OBP.

    Note that hitting into a fielder's choice doesn't help a batters OBP either.  

  4. On Base Percentage is calculated by:

    Divide the total of hits, all bases on balls, and hit by pitch by the total of at bats, all bases on balls, hit by pitch and sacrifice flies.

    (H + BB + HBP ) / (AB + BB + HBP + SF)
