If a doctor has written derogatory malicious false notes in your chart damaging your reputation ?

What if in a medical hospital a doctor decides to write in a patients chart that the patient is severely mentally ill etc...and it is not true, and the patient can verify this, and it is damaging

the person's reputation ? Basically if the patient was hysterical due to extreme pain of a 3rd degree burn. The doctor then writes a lot of derogatory stuff about their "mental" state in their chart.

How can you fight back and clear your reputation if a medical doctor in the emergency room has written some nasty notes in the chart and has lied ?

Your medical records are confidential, and cannot be released to anyone without your written consent.  Contact the hospital and request that your records be sequestered, so that other physicians do not have access to them.

You can also write a letter refuting what was said, and ask that it be included in your chart.
