It could mean a lot of things. The #1 thing would be. Go to a Doctor and have it looked at NOW.

It could be acne or something else, why not consult a skin doctor or just a doctor first?

Is it paiful/ a bump you can feel on your head means it has to be outside the skull. I would suspect a infected hair follicle or if it's behind your ears, a lymph node. Get a friend to look at it with a flashlight and let us know what it looks like.

That happened to me.

You probably have a tumor or a growth in your skull.

Get it checked out before it takes up 3/4 of your head.

I had a tumor in my skull and they had to remove a golf ball sized chunk of my skull from the back of my head.

(PS: The 3/4 of your head thing is a reference toward something the doctor told me. He said it's good I looked into it soon because if you ignore it, it could take up as much as 3/4 of your head.)
