5x Q.D. or 5X q.d.  I don't think it matters much about the capitalization.

5 in Latin is quinque, but since q.i.d. is used for 4 or quater, I've not seen any other abbreviation to denote 5.  

Quaque die in Latin is translated as "one day" which stands for Q.D.  5x, is 5 times.  "q.d., qd, QD" in prescription terms is once a day, thus 5x q.d. = 5 times a day.
