  1. I know in OHIO, Insurance follows the vehicle.  If you drive her vehicle and have an accident, her coverage would be primary, if you exhaust her liability coverage.  Your coverage would be secondary and pick up after hers.  Be careful, not all policies extend to cover other drivers that are not listed on the policy.  It would be best to have her call her agent/company to verify.

  2. Insurance follows the car.  Your insurance policy will not cover the accident you had in your girlfriends car.  Automobile insurance policies are limited to scheduled owned autos.  You couldn't, legally, insure your girlfriend's or anyone else's car even if you wanted to.  You must be an owner of the vehicle to insure it, this would be verified by your name being on the title.  (Different rules apply for rented vehicles)

Your girlfriend's insurance will pay, minus any deductibles, her premiums will go up (assuming that it was your fault) and she may also be canceled.

Insurance companies don't like to insure regular drivers or household residents that they know nothing about.

  3. this would be covered under your girlfriends policy.  you would be assessed points for the accident on your policy and your rates would go up.  her rates may go up depending on how much her company paid out for the claim

  4. It depends on her policy. Some policies could be restrictive about other people driving the car. Ordinarily, you would be covered under her policy, if you borrowed her car with her permission. If you don't have her permission, you are not covered. The insurance follows the car, not just the driver. Best thing to do is call the insurance agent and confirm the coverage.

  5. This is not a nice predicament for you as a boyfriend.  Wrecking someones car is a major claim against the insured.  Now she has a claim and her insurance may increase; or she won't get good rates for another 3-5 years.  You owe her.  But her insurance will pay if she allowed you to drive her car.  Your insurance may be liable if she knows how to go about claiming.
