  1. The specifics are impossible to predict, but you should be aware that in the US, there was no Medicare until the 1960's, and there was no private health insurance to speak of before World War II. Many doctors did not have a good income, but others did.

    There is also now a small number of physicians practicing "boutique medicine." They don't deal with insurance at all. It's all cash-on-barrel-head. Doctors are smart people, so left to themselves, they'll figure out something. But the government will come up with some intervention. The voters simply will not tolerate being told they can't have what they've become accustomed to.
