  1. IDK why don't you change YOUR last name to match the baby's instead?

Why does the child always be the one who has to lose their identity?

    Sheesh.

  2. as long as its ADOPTED and not just custody for fostering purposes. If you are fostering, then the child still belongs to the state. But if the child is adopted you can change the name to yours.

  3. you must either adopt the child or the bio parents have to apply for permission to change any part of the name. Just custody or guardianship does no give you the right to do that.

  4. If you legally adopt the baby you can change the baby’s name to anything you want. You are now the legal parent. Now if you have custody of your child can your change the name with out the approval of the baby’s other parent?   I really don’t know but I don’t think it would be morally right to do that.
