Yes, but it's all part of the package and the free calls compensate. You get up to an hour to any BT landline (hang up and redial if you want to talk for longer), so it's got to be worth it.

Yes you do, the hub phone is pretty cr@p though and the free calls are only to landlines in the evening and weekends.

You pay line rental for the maintenance of the pair of wires that your broadband (and your traditional PSTN line) uses from your house back to the exchange

yes, the home hub gives u a second line on the one line rental thats all, you can use both at the same time though
