  1. Not sure what you mean when you ask if it would "go to DMV."  When you miss a court date, the court issues a warrant for your arrest.   After you get arrested, you are eventually brought to the court for trial on the original violation and on the new crime of failure to appear.   Only after you are convicted does the DMV records show  a conviction.


  2. Should be on the back of the citation. Generally it's the General District Court or Traffic Court but it is normally on the back of the ticket. You should get something in the mail shortly or a uniform at your door....if you're unlucky.

    Surf the state police website - it may have more info on tickets than local law enforcement..
