If my medical insurance has not paid my claims or whatever reason, how long does the provider have to bill me?

There's no time limit.

However long they want. Some insurance plans take three weeks to a year to process a claim, and the provider can bill whenever they want.  I once got a claim processed THREE YEARS after the date of service and billed the patient their co-insurance. Since I had proof that we just got notified on the claim processing, there's nothing that can be done to fight it.
