  1. There's no time limit.


  2. However long they want. Some insurance plans take three weeks to a year to process a claim, and the provider can bill whenever they want.  I once got a claim processed THREE YEARS after the date of service and billed the patient their co-insurance. Since I had proof that we just got notified on the claim processing, there's nothing that can be done to fight it.
