  1. Speaker of the House would take over if both the President and VP were dead. If just the VP died then the President would nominate another VP and have him confirmed by the Congress.

  3. The President would choose and nominate a new Vice President. Sort of the way Ford ended up as President.

  4. If the president is still alive then no one, he gets to appoint a new VP. that's how G. Ford became president with only a single vote. If both the president and the VP die (or are impeached) then it falls to the speaker of the house.
