As far as I know, it won't do anything to the optic nerve - but it'll progress until it will be hard for you to see - they need to take out the internal lens which is nice because it's also like getting laser surgery - so your prescription will be better, often as much as not needing any glasses.

  2. Cataracts will not affect those parts of the eyes. It is the lens in the eye behind the pupil that is affected. It will affect your vision to some degree depending on how far along it is. As it develops you may become more near sighted, which is very common. You may also experience night time glare. Most doctors will wait until absolutely necessary to do the surgery, mainly when you reach a certain visual acuity and can no longer be corrected better than that. Also, insurance will dictate the requirements for them to pay for the surgery, that is why you can't have it done in the early stages. Most of the time the affects on the vision are minimal early on. Depending on the type and existent you have, it could take years to be ready. I would imagine you will be seen yearly for a cataract check and eye exam.
