It's pretty simple and a very literal definition. At any point has there been a p***s inside the female? Yes? Not a virgin. No? Virgin. Easy.

The only complication I could see would be raping a virgin, in which case I personally believe that someone who is raped when they're a virgin has not actually had consensual intercourse and if they so choose to remain a virgin, they can. That's more of an emotional situation and it's complicated. But it's really the only exception I can think of.

Keep in mind we are talking about only heterosexual s*x, here, also. Just throwin' that out there. s*x is obviously going to be a different activity for two females or two males.

I have heard that in some cultures fathers are required to deflower their daughters.  If the father dies, then an uncle assumes the duty of sacrificing the virginity.  Just like you say.  I think an annulled marriage restores virginity.
