Someone in my school broke an arm and he went to the ER. He said it cost him 300$ but shouldn't it be free. He said he dosent have insurance. If your an illegal immigrant than is that the only way of not having insurance)

You can get health insurance if you choose to. If you choose not to get health insurance then you dont have to have it. It is also not illegal its not manditory. Its good to have it, but its like a life decision you make. (another one of those)
