if you lived in a surrounding area? I was born in Evanston, Illinois for example, but can I get it from the city hall in Chicago, or do I have to go to Evanston? Also what information do I have to bring with me to obtain my birth certificate (I'm 17 yrs. old) do I need one of my parents?

If someone can provide me a number to call and ask them, I'll vote you as best answer.

  1. usually the records department can print them up with proper identification, but it does depend on the state

  2. It took me a few minutes.  Google 'Evanston, Il' to find the city's website.  Click on 'city services'  (there are a LOT of city services), then click on 'Birth and Death certificates'.

    Or just click on this.

    http://www.cityofevanston.org/department...

It shows you how to get a birth certificate.  You have to be over 18.  You have to pay $10. And you have to have two forms of ID.

    There you go.

  3. Sometimes birth certificates are with the probate court and sometimes with Vital Records  that is where it is most of the time rather than city hall, though it is in the same building most of the time.  It could be a county vital records rather than city.  You don't have to go there anymore though.   You can do it by email through the web site of the vital records or vital statistics office of the birth location.  I've done that with both my husband's and my own.  You do have to get a form  usually and  you have to pay a fee - quite often about $8 but can be around $15 as well.  That is for certified copies and each office varies somewhat but is reasonable fee. Additional copies are cheaper so you can get more if  you wish.  Can usually pay by credit card.  If need a check, you will print off a form and fill it out. You must have the date and place of birth and your name.  There may be other requirements by the office.  Check on the internet for the town and birth records and you should be able to find it.  I think you shuld check county records as well.

They will send it in the mail to you.  I can't remember what all they require but they may require a driver license or some form of ID.  For parents records, are they deceased or living?  if living, hey may have to get their records.

  4. I'm sure Evanston has a city website with phone numbers.  You can call them and ask.  If they don't have  it, they can tell you where to look.

Each state is different.  In some states, only the counties keep such things, in others, there is  a separate vital records office.

  5. Usually. Also check with the hospital.
