Tar.  I am not sure that tar is the primary cause of lung cancer, I believe it is other constituents that are the major causes, although having all those polycyclic aromatic hydrocarbons, many of which are known carcinogens, inside your lungs for the long term is probably a source of increased risk.  

    But tar does cause emphysema and other lung ailments-coats all the little hairs and the surface cells where air exchanges with blood, considerably decreasing how well the lungs work.

    if you smoke, you have certainly coughed up dark phlem from time to time.  The tar is a primary reason people get "smoker's cough".  Trying to clear their lungs of the goop in there.
