I'm a referee for the BCA Pool League and the answer is, yes, you would lose if you both pocketed the 8-ball and the cue in the same shot.  Your opponent would then have ball in hand anywhere on the table.  The only time ball in hand is restricted to behind the head string is when the table is open (suits have not been decided).  This is contrary to many bar rules, but that's how the pros play.

  12. Per your question, yes you lose if you scratch when you are "on" the 8 ball, and, the eight ball, at the same time enters a pocket or leaves the table.

    However, if you scratch when you are "on" the 8 ball, and, the 8 ball  neither enters a pocket nor leaves the table, you do not lose. You turn at the table ends and your opponent has ball in hand behind the line.

    These rulings exclude what would happen if such occured during the "break" shot. (If this occurs during the break you do not lose)
