    Previous passport with the visa

    Three (3) identical colored photos 1.77" × 1.37" (3.5 × 4.5 cm) recently taken, front view, with a white background. (No eyeglasses or sleeveless attires please.) Poor quality digital photographs not allowed

    Proof of Philippine citizenship (green card, INS notice, employment authorization card, visa, etc.)

    Additional requirement for minors: notarized parental consent to travel signed by both parents

    Additional requirements for married female individuals who wish to change their maiden names to their married names:

    Marriage Contract (see guidelines regarding registration of marriage)

    Divorce Decree (see guidelines regarding registration of marriage)

    One (1) colored photo 1.77" × 1.37" of husband and wife

    Fees (Payable in cash or money order)

    $50.00 Passport Booklet

    $20.00 Amendment fee (for change of name requests) in lieu of Passport Booklet Fee

    $25.00 Report of Marriage (for change of name requests)

    Application Form (notarized for MAILED applications)

    For married female individuals changing their surnames:

    Report of Marriage Forms, 3 sets including attachments (notarized if MAILED)

    Amendment Form in lieu of Application Form (if passport is still valid, notarized if MAILED)

    CFO Info sheet (if married to foreigner)

    Data page of the previous passport, i.e. pages bearing

    Photo of passport bearer

    Personal information

    Last page bearing signature of signing officer

    US Immigration Arrival Stamps

    US Visa

    Proof of MIDDLE NAME

    For married female individuals changing their names

    Marriage Contract (4 sets)

    Photocopy of husband's passport (3 sets)
